Please use the appropriate medical form from the two listed below.  This should be completed and preferably emailed to school before or on the day the medication is to be required.

AM1 –  for all general medicines eg ear drops, skin creams and including antibiotics,

AM5 –  for medication which is prescribed for more than 8 days eg. Inhalers (not for children who have a care plan provided by medical professionals)
